0165 (2)(a) The licensor observed a dime-sized object located beneath the Pack 'n Play mat, posing a potential choking hazard if the area is not properly cleaned or inspected before being occupied by a child. 0165 (5)(e) The licensor observed a surge protector covered with cloth material, creating a potential fire hazard due to restricted ventilation. Additionally, the surge protector was placed on top of a mini refrigerator, making it easily accessible to school-age children and increasing the risk of electrical injury or tampering.
